Risk Register & Mitigation
The identified risks to pilot success and enterprise expansion, with likelihood, impact, mitigation strategies, and owners.
Risk register
ObservedMembers discontinue therapy after initial weight loss, eroding long-term clinical and financial outcomes.
Manager-submitted OT, temp, and absence data is not submitted on time or lacks required detail for validation.
Medical claims data has a 60-90 day lag, delaying cost avoidance validation and Gate 4 decision.
Current care team cannot support enterprise-wide rollout without additional hiring and training.
Daily check-in participation declines over time as novelty wears off.
GLP-1 medication supply constraints or price changes affect program cost and continuity.
Unauthorized access to PHI or PII through system vulnerability or insider threat.
Enterprise expansion budget is not approved by leadership, delaying or preventing scale-up.
Contingency planning
If Gate 4 criteria are not met, the steering committee has two contingency options: (1) extend the pilot by 3 months to allow claims runout and additional data validation, or (2) proceed with a limited Wave 2 expansion (1,500 members instead of 3,000) to validate scaling while controlling risk. Either contingency requires steering committee approval and a revised timeline.
Employee Privacy Protected by Design
Employer reporting is designed to present aggregate workforce and health-program insights without exposing individual clinical records. Small-group reporting restrictions help reduce the risk of employee identification.
Aggregated information is not described as HIPAA-de-identified unless applicable de-identification requirements have been satisfied and verified.
